How do I request my medical records?
Call 845-471-3500 and ask for records. The office will tell you what it needs in writing and how long the request takes.

Where are the new-patient forms?

How do I book an appointment?
New patients can book online. If you are already a patient, call 845-471-3500 — the office schedules against the clinical calendar directly, which is faster and avoids double-booking.
What if I need care after hours?
For a medical emergency call 911 or go to your nearest emergency department. For non-urgent questions outside office hours, call 845-471-3500 and follow the instructions on the recording.
